I've narrowed the problem down to multiple page duplex printing. For troubleshooting purposes I discovered that the tray the paper came from didn't matter, so I used the manual feed tray to troubleshoot some more.
If I put up to 3 pieces of paper in the manual feed, the duplexing works fine. 4 or more causes a jam in the area where the 4 belts move the paper along. The jam consists of a partially printed page with non-set toner and a new page. The new page is about 3 inches down from the partially printed page. Interesting is that the two pages are attracted together via a static charge.
Some investigation on searching makes mention of a sensor in the duplexing shelf. I've dropped the duplex shelf and can't see anything wrong there, but don't know where the sensor is.
