4250 here that is misbehaving in several different ways.
New swing plate assembly has been installed. I'm testing this with two different fusers, neither of which have a broken clip. All rollers have about 90,000 pages printed with them and are still rough and not glossy/smooth.

1. Instead of a print beginning at the top of the page it begins printing anywhere from 1/4" to 3.5" down the page.

2. It gets a 13.20 error. Paper will be sticking out the back of the fuser (with rear door open) about 1.5". A 2nd page will be either *just* past the transfer roller or with the trailing edge sitting on top of the transfer roller.

This happens when printing from tray 1, 2, or 3.

3. 41.3 UNEXPECTED SIZE IN TRAY X error. So far I've only seen this from Tray 2, though it is very intermittent.

I've seen all 3 of these issues manifest on their own and in conjunction with another issue. Sometimes it'll print a single page without jamming but the print won't start at the top. Or it will print with the top at the top, but then jam on the 2nd page. Or it will print one page with the top somewhere other than the top and then give the 41.3 error.

I managed to get about 6 normal pages to print from the manual tray, then it jammed every time. Then for a bit it would give 41.3 error after every page. Currently it jams at the rear door about 90% of the time.

From what I understand after reading countless forum posts here and elsewhere is that this could be a DC Controller issue or might be a problem with the top sensor (PS103 and yes I know it is very unlikely for it to be the sensor itself).

 The only thing in common here is that a solenoid actuates and releases the paper. What is obviously happening is that it is actuating too early (as evidenced by the downward print shift) and the paper's arrival at the sensor is premature. That triggers the 41.3 error. One could blame it on a sticking solenoid releasing the paper before it's supposed to, but since there is one for each tray, the likelihood that 3 went bad simultaneously is infinitesimal. Sensors don't control paper release, and as we all know...they never go bad. The DC controller has command of everything, so that would be the most likely culprit. I've never seen this behaviour so that would be my best guess.
